Applying for an Instruction Permit

Eligibility - To be eligible for an Instruction permit, a person under 18 must: 

• Be at least 15 years and 6 months of age. 
• Be a Wisconsin resident. 
• Pass the knowledge test and sign test with 80% or greater passing score.
• Pass the vision screening of 20/40 vision with or without corrective lenses.
• Be physically and mentally qualified.
• Be enrolled in behind-the-wheel training that will start within 60 days of application for the permit.
• Be enrolled in a State of Wisconsin Approved Drivers Education Program.
• Have an adult sponsor sign the application.

Instruction Permit Application

The following must be provided at the time of application for an instruction permit:

• MV 3001 Driver License Application - The application should be fully completed. Signed by the School.
• Proof of name and date of birth - An acceptable document with full name and date of birth. A Certified birth certificate or the most commonly used document.
• Proof of identity - a document with the name and photo or name and signature of student as a social security card or student ID. The parent or guardian may also identify the student as long as the parent has a WI driver license or ID card.
• Social Security Number - it is necessary to have the card along, but the number must be known to complete the driver license application.
• Sponsor Certification if under 18 - An adult sponsor must sign the sponsorship certification in Section D of the MV 3001, and the signature witnessed by a DMV representative.
